Until recently, patients with relapsed or refractory diffuse large B-cell lymphoma (DLBCL) who are ineligible for stem cell transplant had no curative options. Today, nearly every patient in the second-line setting can be treated with curative intent. Dr. Josh Brody and Dr. Jeremy Abramson walk through the options: CAR T-cell therapy, bispecific antibodies, antibody-drug conjugates (ADCs), and combinations of these. They cover how to choose the optimal therapy for transplant-ineligible patients, especially when CAR T is not an option or if the disease is progressing rapidly. They also discuss creative bridging strategies, where MRD testing fits today, long-term infection risk with bispecific antibodies, and practical tips for sharing care between academic centers and community oncologists. The views and opinions expressed herein are those of the author(s)/faculty member(s) and do not reflect the official policy or position of their employer(s) or the Society of Hematologic Oncology.
