Maryland StARS State: Presentation and Discussion

Description: Learning Collaborative attendees will gain an understanding about how the Maryland StARS Project standardized and connected data across statewide dementia, aging, health, and social service systems to strengthen bidirectional communication, referrals, and care coordination between community-based organizations and healthcare providers. The session will also highlight how integrated data can help quantify health outcomes and cost savings while enhancing the Maryland Department of Health’s dementia dashboard with information on dementia prevalence and the service needs of people living with dementia and their caregivers.
