CO-45 Denial in Medical Billing Explained! Learn the meaning of CO-45, understand a real $150 claim example, and discover how an AR caller should investigate, verify, and handle CO-45 adjustments step by step in US healthcare medical billing.
Are you a medical billing beginner, AR caller, denial management specialist, or someone preparing for a US healthcare RCM interview? This practical training video explains CO-45 in simple English, including contractual adjustments, write-offs, and the correct steps to take when an allowed amount appears incorrect.
WHAT IS CO-45?
CO-45 is a Claim Adjustment Reason Code (CARC) used when a charge exceeds the applicable fee schedule, maximum allowable amount, or contracted or legislated fee arrangement.
CO = Contractual Obligation
45 = Charge exceeds the applicable fee schedule, maximum allowable, or contracted or legislated fee arrangement.
CO-45 is commonly a contractual adjustment, not automatically a claim denial requiring an appeal. Always review the EOB or ERA and claim details before deciding what action to take.
REAL CLAIM EXAMPLE
Billed amount: $150
Allowed amount: $100
CO-45 adjustment: $50
Calculation: $150 − $100 = $50
If the adjustment is valid under the applicable contract, it is generally posted as a contractual write-off according to company policy and generally cannot be billed to the patient under the applicable contractual terms.
HOW TO HANDLE CO-45: FIVE STEPS
1. Check the EOB or ERA.
2. Verify the contractual amount or fee schedule.
3. Check the claim status.
4. Decide whether to adjust, investigate, or escalate.
5. Document your findings and the next action.
If the allowed amount appears incorrect, investigate the provider contract, fee schedule, CPT code, modifiers, units, date of service, and applicable payer rules. Contact the payer if necessary and follow the appropriate correction, reconsideration, or appeal process within the applicable time limit.
WHO SHOULD WATCH?
• US healthcare medical billing beginners
• Medical billing and AR calling trainees
• AR callers and denial management specialists
• RCM professionals
• Candidates preparing for medical billing interviews
GOLDEN RULE: Never take action just by seeing a code. Understand the adjustment, verify the facts, and then take the correct action.
