✔ The first and most common cause in active people under forty is patellofemoral pain, which is pain from the kneecap and the structures that connect it to the rest of the knee.
✔ The second common cause is IT band syndrome, which produces pain on the outer side of the knee, typically in runners and cyclists.
✔ The underlying cause is almost always weakness in the hip abductors and gluteus medius, combined with training load increases that have outpaced adaptation.
✔ A large proportion of people with meniscal findings on MRI have no symptoms at all.
