Your first wife may still be named on your work retirement account. If you never changed that form, the people who pay that money out are required to follow the paper they hold, not your will.
You did the right thing at the bank. You asked for the form and you wrote your children on it. That form is real and it works — on one account. In this video I go through the four accounts it never reaches, and the one limit that changed in 2024 without a letter to anybody: the account whose named person died before you did, the account where your shares are kept, the account for the small business you run in your own name, the work retirement account still holding a name from a marriage that ended, and why naming a sixth child adds nothing.
