THE SHORT ANSWER
The decision to intubate is clinical, not a number on a blood gas. The four classic indications are failure to oxygenate, failure to ventilate, failure to protect the airway, and an anticipated clinical course that will lead to one of those three. Assess the airway before you commit — a difficult airway recognised in advance is a manageable problem; one discovered after induction is not. Prepare drugs, suction and a plan B before the first attempt.
