Does testosterone suppression create a level playing field in women's sports? This episode breaks down what the physiology research actually shows about retained male athletic advantage after transition.
The performance gap starts earlier than most people assume. Nine-year-old boys post roughly 9.8% faster sprint times, 16.6% faster mile times, and 13.8% stronger grip than nine-year-old girls, and similar gaps show up in six-year-olds — all before puberty. Testosterone exposure in the womb and a "mini puberty" between one and six months of age both leave marks. Then puberty drives divergence through growth hormone and a much larger IGF-1 response, producing permanent differences in bone structure, limb length, lung and heart size.
What suppression actually reverses is smaller than the debate assumes. A pooled look at 13 studies found 3 to 5% loss of total lean body mass from male baseline. Against an average 40% male advantage, that leaves 35 to 37% still in place. Bone density has been shown to persist up to 12.5 years post-suppression, effectively an entire athletic career, and spinal bone mineral density may rise rather than fall. Satellite cell content and myonuclei per fiber are retained, which is the mechanism behind muscle memory.
