Please note that requirements may vary depending on your application type and personal circumstances. Always confirm the latest checklist with the accredited travel agency or the Embassy of Japan. 1. Duly completed visa application form with original signature 2. PSA Birth Certificate (may not be required for a subsequent application, depending on the applicable requirements) 3. Previously used Japan visa, if available 4. Two (2) recent colored photos, 4.5 cm × 3.5 cm, with a white background 5. Bank Certificate 6. Bank Statement 7. For employed applicants: Original Certificate of Employment (COE) with original signature, photocopy of Company ID, and Income Tax Return (ITR) 8. For business owners: Photocopies of DTI Registration, Mayor’s Permit, BIR Certificate of Registration (COR), and ITR 9. For licensed professionals: Photocopy of PRC ID 10.
