Elaine Herzberg was the first person killed by a self-driving car. On 18 March 2018 in Tempe, Arizona, an Uber test vehicle detected her and did not brake.
On March 18, 2018, an Uber ATG test vehicle — a modified Volvo XC90 driving itself on a test loop in Tempe, Arizona — struck and killed Elaine Herzberg as she pushed a bicycle across N. Mill Avenue. This self driving uber car killed a pedestrian in what became the first known autonomous vehicle accident to take a life. The NTSB investigation found the automated driving system detected her 5.6 seconds before impact but never classified her as a pedestrian, because its design did not include consideration for people crossing outside a crosswalk. Every time it changed its mind about what she was, it discarded her tracking history. When it finally declared an emergency, 1.2 seconds out, a deliberate feature called action suppression held the brakes for one full second, with no alert to the operator. The Volvo's own factory emergency braking had been switched off while the test system drove.
