In this episode of unPAUSED, Dr. Mary Claire Haver sits down with Dr. Eleonora Teplinsky, board-certified medical oncologist specializing in breast and gynecologic cancers and author of Beyond the Pink, to take on the dogma that has left millions of women suffering: if you’ve had breast cancer — or even a family history of it — you can never take estrogen. Dr. Teplinsky says it plainly: estrogen does not cause breast cancer. It can fuel the growth of cells that have already transformed — a crucial difference — and that distinction, lost in 2002’s headlines, is why the real number behind the Women’s Health Initiative panic was eight additional cases per 10,000 women per year, in one specific hormone combination that is no longer standard practice. Dr. Teplinsky walks through who genuinely isn’t a candidate for systemic hormones and when “no” really means “not right now,” the low/medium/high framework she uses for recurrence risk, and the exact three-sentence script to take into an appointment with a dismissive oncologist — plus why, for some survivors, the recurrence risk from alcohol may outweigh the risk from hormones. They dig into the 20-year-old HABITS and Stockholm trials and why that data can’t govern treatment in 2026, her WISH survey finding that 73% of survivors never had a single sexual-health conversation with a provider, vaginal estrogen and DHEA for the 60%+ of survivors with genitourinary symptoms (safe even on tamoxifen or an aromatase inhibitor, with shared decision making — and exactly how to apply it), aromatase inhibitors as “menopause on steroids,” and the quiet headline: in 2026, the NCCN guidelines moved testosterone for breast cancer patients from “contraindicated” to “use with caution” — a door opening.
