Progressive multifocal leukoencephalopathy, or PML, is a subacute demyelinating disease of the central nervous system caused by reactivation of JC virus in immunosuppressed patients. It is an important MRCP and clinical medicine topic because it can present with progressive focal neurological deficits and may mimic several other CNS conditions.
In this video, we simplify PML from the basics, including how JC virus causes oligodendrocyte injury, why demyelination occurs, and how the condition presents clinically. We also discuss the key MRI findings, CSF testing, important differentials, and the basic principles of management, including immune restoration and recognition of PML-IRIS.
We will cover:
Definition of progressive multifocal leukoencephalopathy
JC virus reactivation and pathophysiology
Risk factors including HIV, malignancy, transplant, and immunomodulatory drugs
Clinical features and neurological deficits
MRI and CSF diagnosis
Differentiation from MS relapse, CNS lymphoma, toxoplasmosis, HIV encephalopathy, vasculitis, ADEM, PRES, and drug-related leukoencephalopathy
Basic management principles
PML-IRIS and steroid use in severe inflammatory cases
This video is designed for medical students, MRCP candidates, junior doctors, and anyone trying to understand progressive multifocal leukoencephalopathy in a simple, clinically relevant way.
