The condyle presses into the back of that joint, close to your ear, where there are nerves and blood vessels. That pressure is what creates TMJ pain. So the fix is not just moving the jaw forward. It is moving it into the right three-dimensional position, away from that sensitive area, and securing it in a comfortable new bite.
