In this recorded Neurology Grand Rounds presentation, Director of the Neuro-Immuno-Oncology Program and Professor of Pediatric Neuro-Oncology at Stanford Medicine Dr. Michelle Monje and Dr. Jasia Mahdi (Co-Primary Investigator) discuss their clinical trial for GD2 CAR T-cell therapy in diffuse midline gliomas (DMGs). Dr. Monje introduces the topic of diffuse midline gliomas (DMGs), particularly focusing on diffuse intrinsic pontine gliomas (DIPGs). She describes these as fatal malignancies with a median survival of only 10-11 months for DIPG. Dr. Monje explains that these tumors integrate structurally and electrically with neural circuits, making them challenging to treat. Dr. Monje then explains that her laboratory identified GD2 as a promising target on the surface of DIPG cells. This disialoganglioside is highly expressed on tumors with the H3K27M mutation, which is present in over 80% of DMGs. The discovery led to collaboration with Dr. Crystal Mackall to develop optimized GD2-targeting CAR T-cells. Dr. Mahdi then concludes the lecture with exciting updates on the clinical trial.

Learning Objectives:
1. Explore different administration routes and dosing regimens for GD2 CAR T therapy to improve efficacy and tolerability.
2. Discuss the importance of studying the tumor microenvironment and factors contributing to tumor growth and resistance to develop more durable responses.
3. Highlight the need for a multidisciplinary approach in delivering GD2 CAR T therapy safely, involving collaboration with neurology colleagues.
4. Identify the long-term outcomes of patients who showed significant responses, such as the patient with a complete response sustained for over 20 months.
5. Investigate the role of lymphodepleting chemotherapy in inducing more durable responses with ICV infusions.
