The FDA agreed the trial hit its primary endpoint — then rejected the drug anyway. Dr. Jennifer Lyerly fully expected approval explains why the data changed her mind. In October 2025, low-dose atropine's FDA approval felt like a formality to most of optometry. It didn't happen. In this conversation we go through the Phase 3 STAR study — the largest low-dose atropine trial ever run, 847 children, three years — and talk honestly about what it does and doesn't show. WHERE THINGS STAND (as of publication): FDA issued a Complete Response Letter on October 23, 2025, citing effectiveness rather than safety or product quality. Sydnexis filed a Formal Dispute Resolution Request, and in July 2026 the FDA agreed to convene an advisory committee to review the application. A meeting date has not been announced. The same molecule was approved in the European Union in June 2025.
