Under NHM -Meghalaya's commitment to Universal Health Coverage, this best practice model deploys ASHAs, ANMs, and health teams to conduct simultaneous TB and NCD screenings in geographically isolated communities. Moving beyond facility-based care, the initiative leverages convergence of disease programs to reduce missed cases, lower the dual burden of communicable and non-communicable diseases, and strengthen community trust, making comprehensive health screening accessible, affordable, and actionable at the last mile.
