On January 5, 2024, a door-sized panel blew out of a nearly new Boeing 737 MAX 9 at almost 15,000 feet over Portland, Oregon. All 177 people on Alaska Airlines Flight 1282 survived. The NTSB investigation traced the hole back to Boeing's Renton factory: four bolts removed during a repair, never put back, and no paperwork to catch it.
This breakdown is built entirely from the official NTSB investigation: the final report, board meeting, press briefings and investigative photos and video.
