We know what happens during labour in huge detail, but for decades nobody could say what actually starts it. It's especially puzzling in humans, because progesterone (the hormone that keeps the uterus relaxed, like a brake pedal) stays high right through labour. In this video I look at a new study from Stephen Lye's lab at Mount Sinai Hospital in Toronto. It suggests that immune cells called macrophages move into the womb muscle near the end of pregnancy. They build tiny bridges called tunnelling nanotubes and hand over their own mitochondria, the cell's "power stations". This fuels the muscle for contractions. It also switches on an enzyme that breaks down progesterone inside the muscle, which takes the brake off.
