2019 06 14 Vancomycin Dosing Review of Current Literature and Recommendations for Monitoring

This presentation was presented by Erin Rachmiel, PharmD, BCPS – an Antimicrobial Stewardship Pharmacist at the Washington University School of Medicine. Dr. Rachmiel reviews vancomycin kinetics and guidelines that were current at the time of presentation. Additionally, much of the presentation reviews dosing and monitoring recommendations with a focus on area under the curve (AUC) vs. trough monitoring. The presentation concludes with a discussion of AUC dosing tools and some considerations for institutions that may be considering a change to AUC monitoring of vancomycin.
