What’s one uncomfortable truth about your field? That simple question opens up one of the most revealing conversations in this episode of Between Bites with Dr Hosnieh Fathi. From food waste and the wellness industry to pharmaceutical research, misinformation, burnout and the pressures facing doctors, the panel explores some of the realities that are often far more complicated than social media makes them appear. Chef Vince Kelly begins with the uncomfortable scale of food waste in hospitality — and why simply composting surplus food does not solve the wider problem. The conversation moves into planning, customer expectations, technology, using more of the ingredients we already have, and changing the way both kitchens and consumers think about waste. Vanessa Sturman turns to another uncomfortable reality: wellness is itself a major commercial industry. While pharmaceutical companies are often criticised for being commercially driven, wellness products, programmes, retreats and online health advice are also being sold to consumers every day. The question becomes less about choosing one “side” and more about learning how to distinguish good evidence from persuasive marketing and misinformation. Dr Hosnieh Fathi brings her experience from both the NHS and pharmaceutical clinical research. She discusses the tension at the heart of pharma: it is a commercial environment, but that commercial investment has also enabled enormous advances in medical research, treatment and drug development. Rather than presenting pharmaceutical medicine and lifestyle health as opposing worlds, the discussion asks whether both can exist together. You can take medication when it is needed and work on nutrition. You can benefit from medical treatment and improve movement, sleep, stress management and other health behaviours. Two things can be true at the same time. The conversation then takes another turn when Dr Safia Debar offers perhaps one of the most personal uncomfortable truths of the discussion: “There are a lot of wounded healers.” Healthcare professionals are expected to care for others while working within environments that can place enormous demands on their own nervous systems, sleep, emotional health and recovery. How can clinicians properly hold space for patients if they are never given the tools or space to regulate themselves? Should healthcare professionals be taught more about recovery, difficult conversations, nervous-system regulation and emotional wellbeing? And how do we build healthcare systems where those caring for others are also able to care for themselves? This episode is ultimately about moving beyond false choices. Medicine or lifestyle. Pharma or wellness. Treatment or prevention. Commercial interests or genuine benefit. Reality is rarely that simple. The challenge is learning to hold more than one truth at the same time — while staying critical of misinformation, commercial influence and overly simplistic health claims.
