El Faro sailed into Hurricane Joaquin carrying 33 people. The disaster was not one mistake — it was a chain of warnings, decisions, information gaps and system failures.
This documentary reconstructs El Faro's final voyage using NTSB evidence, the voyage data recorder transcript, hurricane advisories and the official investigation.
On September 29, 2015, El Faro left Jacksonville for San Juan as Tropical Storm Joaquin developed in the Atlantic.
Over the next two days, the storm intensified dramatically. Forecasts changed. Weather information reached the ship through different systems and at different times. Alternative routes existed. Calls were made during the night. By the morning of October 1, El Faro was inside a Category 3 hurricane.
At 06:13, the captain reported:
“I think we just lost the plant.”
This episode follows the warning chain from departure to the final emergency — and then examines what investigators concluded, what changed after the loss, and which links in the chain might have been broken.
