On the night of 14–15 February 1982, the Ocean Ranger, once the largest semisubmersible drilling rig in the world, capsized and sank in a winter storm on the Grand Banks of Newfoundland. All 84 men aboard were lost. The investigations traced the loss to a chain of small things: a window in the ballast control room, a control panel that failed when it got wet, a manual backup almost nobody had been trained to use, open chain-locker openings high on the columns, and a sea so cold that there was almost no time to be saved. The Royal Commission later concluded that, had the damaged ballast system been isolated correctly that night, the rig and its crew would have survived the storm. This documentary follows the warning chain behind that finding.
