🚨 Why does phosphate rise, PTH increase, and bone disease develop as CKD progresses? Master the pathophysiology, laboratory patterns, and treatment of chronic kidney disease–mineral and bone disorder (CKD-MBD) in this high-yield nephrology board review! 🧠 1. WHAT IS CKD-MBD? CKD-MBD encompasses systemic abnormalities of mineral and bone metabolism caused by CKD, including: Abnormal calcium, phosphate, PTH, and vitamin D metabolism. Abnormalities in bone turnover, mineralization, and strength. Vascular and other soft-tissue calcification. 🔬 2. PATHOPHYSIOLOGY: THE FGF23–KLOTHO–PTH AXIS As kidney function declines: 1️⃣ FGF23 rises early in response to phosphate retention and other stimuli, increasing urinary phosphate excretion in surviving nephrons and suppressing calcitriol synthesis. 2️⃣ Calcitriol decreases because the diseased kidneys have reduced capacity to produce active vitamin D. Intestinal calcium absorption falls. 3️⃣ Phosphate retention worsens as CKD progresses, contributing to secondary hyperparathyroidism. 4️⃣ PTH increases in response to phosphate disturbances, reduced calcium availability, and reduced calcitriol. Parathyroid hyperplasia may develop. 🎯 Board pearl: FGF23 often rises before serum phosphate becomes elevated. A normal serum phosphate does not exclude early CKD-MBD. 🧪 3. LABORATORY PATTERNS Early CKD: FGF23 and PTH may rise while serum phosphate and calcium remain normal. Advanced CKD: phosphate often rises, calcitriol falls, and secondary hyperparathyroidism worsens. Calcium: may be low, normal, or affected by treatment and other factors. Interpret trends together rather than relying on one isolated laboratory value. 🦴 4. RENAL OSTEODYSTROPHY: THE BONE PHENOTYPES High-turnover bone disease: osteitis fibrosa cystica due to excessive PTH activity. Adynamic bone disease: low bone turnover, associated with factors such as excessive PTH suppression, diabetes, and older age. Osteomalacia: defective mineralization. Mixed uremic osteodystrophy: features of both high turnover and defective mineralization. Bone biopsy with histomorphometry is the gold standard for defining renal osteodystrophy when the result would change management. ⚠️ 5. VASCULAR CALCIFICATION CKD-MBD also contributes to vascular and valvular calcification. Risk is influenced by phosphate burden, calcium exposure, inflammation, and other cardiovascular factors. Remember: vascular calcification is not simply passive calcium deposition; vascular smooth muscle cells can acquire an osteogenic phenotype. 💊 6. TREATMENT: NON-DIALYSIS CKD vs DIALYSIS Phosphate management Lower persistently elevated phosphate toward the normal range. Counsel on phosphate sources, particularly highly absorbable inorganic phosphate additives. Use phosphate binders when appropriate. Consider calcium load, vascular calcification risk, and the patient's overall mineral profile when choosing a binder. Secondary hyperparathyroidism Correct modifiable factors, including hyperphosphatemia, hypocalcemia, high phosphate intake, and vitamin D deficiency. In non-dialysis CKD, calcitriol and vitamin D analogues are not routinely recommended for everyone; reserve them for selected patients with severe, progressive hyperparathyroidism. In dialysis patients requiring PTH-lowering treatment, options include calcimimetics, calcitriol or vitamin D analogues, and combination therapy. Dialysis patients Use serial calcium, phosphate, and PTH measurements to guide treatment. Avoid excessive calcium loading and overly aggressive PTH suppression. Consider parathyroidectomy for severe hyperparathyroidism that does not respond to medical treatment. 🎯 7. HIGH-YIELD BOARD PEARLS FGF23 rises early and suppresses calcitriol. Advanced CKD promotes phosphate retention and secondary hyperparathyroidism. High PTH suggests increased bone turnover, but PTH alone cannot perfectly define bone histology. Adynamic bone disease is associated with low bone turnover and excessive PTH suppression. Treatment differs between non-dialysis CKD and dialysis; do not automatically prescribe calcitriol for every elevated PTH. Manage calcium, phosphate, and PTH as interconnected abnormalities.
