🚨 A patient develops profound, prolonged hypocalcemia after parathyroidectomy? Think HUNGRY BONE SYNDROME!
In this high-yield nephrology board review, learn why calcium, phosphate, and magnesium can drop dramatically after parathyroid surgery, how to distinguish hungry bone syndrome from other causes of hypocalcemia, and how to treat it.
🧠 1. PATHOPHYSIOLOGY
Severe hyperparathyroidism causes high bone turnover and skeletal demineralization.
After parathyroidectomy, PTH falls rapidly, reducing bone resorption.
Bone formation and mineralization continue, driving calcium, phosphate, and magnesium into the skeleton.
The result is prolonged hypocalcemia, often with hypophosphatemia and hypomagnesemia.
⚡ 2. WHO IS AT RISK?
Severe secondary or tertiary hyperparathyroidism, especially in advanced CKD and dialysis patients
Markedly elevated PTH and alkaline phosphatase
Severe renal osteodystrophy or osteitis fibrosa cystica
Extensive preoperative skeletal disease
🔬 3. CLASSIC LAB FINDINGS
Low calcium: can be severe and prolonged
Low phosphate: due to skeletal uptake, although CKD and phosphate handling can modify the pattern
Low magnesium: may contribute to refractory hypocalcemia
Elevated alkaline phosphatase: often reflects high bone turnover and may remain elevated after surgery
PTH is typically markedly reduced compared with preoperative levels.
🚨 4. CLINICAL PRESENTATION
Perioral numbness and paresthesias
Muscle cramps, tetany, or carpopedal spasm
Prolonged QT interval and potentially dangerous arrhythmias
Severe cases may cause seizures or laryngospasm
💊 5. TREATMENT
IV calcium gluconate for severe or symptomatic hypocalcemia, with ECG monitoring.
Oral calcium supplementation for ongoing replacement.
Active vitamin D, such as calcitriol, to support calcium absorption.
Correct magnesium deficiency.
Monitor calcium, phosphate, magnesium, and kidney function frequently.
Patients with advanced CKD may require individualized calcium and active vitamin D regimens under nephrology supervision.
🎯 HIGH-YIELD BOARD PEARLS
Hungry bone syndrome is classically seen after parathyroidectomy for severe hyperparathyroidism.
Hypocalcemia + hypophosphatemia + high alkaline phosphatase strongly suggests hungry bone syndrome in the right clinical setting.
Hypocalcemia can persist for weeks or months.
Distinguish it from postsurgical hypoparathyroidism: phosphate is often low in hungry bone syndrome but high when PTH deficiency is the dominant mechanism.
