Knocking Ford Mustang 4.6L 2v Complete Engine Teardown | Avoidable Mistake or Random Failure?

Today were tearing down the modular 4.6 2v out of my mustang drift car to find out what failed. It made on a tune with only basic bolt-ons such as an intake and headers around 260hp.
