Infectious esophagitis is not just “Candida plaques.”
An immunocompromised patient may present with odynophagia, dysphagia, retrosternal chest pain, poor oral intake, dehydration, or weight loss — and the cause may be fungal, viral, or occasionally noninfectious.
The key clinical challenge is:
Candida, HSV, CMV, and idiopathic HIV ulcers may all cause painful swallowing, but the biopsy site, treatment, and clinical meaning are different.
In this lecture from Gastroenterology Made Simple, we explain infectious esophagitis through a practical clinical approach:
PLAQUES → Candida
SMALL ULCERS → HSV
LARGE DEEP ULCERS → CMV
LARGE DEEP NEGATIVE WORK-UP ULCER → Idiopathic HIV ulcer or alternative diagnosis
This lecture covers:
• Definition of infectious esophagitis
• Why it occurs mainly in immunocompromised patients
• HIV/AIDS, transplantation, chemotherapy, steroids, neutropenia, diabetes, antibiotics, and esophageal stasis
• Why odynophagia is the key presenting symptom
• Oral thrush: why it supports Candida but does not exclude other causes
• When empiric fluconazole is reasonable
• Why esophageal Candida requires systemic therapy, not topical nystatin alone
• When to perform early EGD instead of prolonged empiric therapy
• Failure to improve after 7 days of fluconazole
• Endoscopic pattern recognition
• Candida plaques and pseudohyphae
• Kodsi classification of Candida esophagitis
• HSV esophagitis and punched-out or volcano-like ulcers
• Why HSV biopsy should be taken from the ulcer edge
• HSV histology: multinucleation, nuclear molding, chromatin margination, and Cowdry A inclusions
• CMV esophagitis in advanced immunosuppression
• Large deep linear or longitudinal CMV ulcers
• Why CMV biopsy should be taken from the ulcer base or bed
• CMV histology: cytomegalic owl’s-eye cells
• Why CMV PCR alone does not prove tissue-invasive disease
• Acyclovir, valacyclovir, ganciclovir, valganciclovir, and foscarnet
• Idiopathic HIV esophageal ulcer as a diagnosis of exclusion
• Why infection and malignancy must be excluded before steroids
• Rare infectious causes: fungi, tuberculosis, bacterial esophagitis
• Why Chagas disease causes secondary achalasia rather than typical infectious esophagitis
• Complications including bleeding, dehydration, malnutrition, stricture, fistula, and perforation
🧠 High-Yield Memory Rule:
Candida = white plaques
HSV = small punched-out ulcers
CMV = large deep linear ulcers
Idiopathic HIV ulcer = large deep ulcer after negative infectious and malignant work-up
⚠️ Major Exam Traps:
• Absence of oral thrush does not exclude Candida esophagitis.
• Topical nystatin alone is inadequate for esophageal Candida.
• No improvement after about 7 days of fluconazole should prompt EGD and biopsy.
• HSV is biopsied from the ulcer edge.
• CMV is biopsied from the ulcer base or bed.
• Positive blood CMV PCR does not prove CMV esophagitis.
• Negative blood CMV PCR does not exclude tissue-invasive CMV disease.
• Idiopathic HIV ulcer is a diagnosis of exclusion.
• Never start steroids for a presumed HIV aphthous ulcer before adequately excluding infection and malignancy.
🚨 Clinical Diagnostic Logic:
IMMUNOCOMPROMISED PATIENT + ODYNOPHAGIA
→ Look for oral thrush and severity
→ If typical Candida syndrome: systemic fluconazole
→ If no improvement by about 7 days: EGD + targeted biopsy
→ If severe, atypical, bleeding, dehydrated, or CMV/HSV/malignancy suspected: early EGD
→ Biopsy according to lesion pattern and suspected pathogen
The central message:
In infectious esophagitis, the endoscopic appearance gives the clue, but the biopsy site gives the diagnosis.
Candida needs brushings or biopsy of plaques.
HSV needs biopsy from the ulcer edge.
CMV needs multiple biopsies from the ulcer base or bed.
