What if the best place to recover isn’t a hospital bed but back in your own home?
The Home Assessment Pathway (HAP) is changing how Health & Social Care Moray supports people at important moments in their lives – like when they’re ready to leave hospital but still need the right support to move forward.
HAP brings strengths-based rehabilitation therapy and reablement support home. It connects a team of professionals – including Discharge to Assess, START, Community Response Team, Pharmacy, Community Nursing, Technology and Volunteers – to work alongside individuals and their families in the place they know best.
At home, we see the person, not a patient. We learn what matters to them, what they can do, and what support will help them regain their strength, mobility and independence.
This approach helps people leave hospital sooner, avoid unnecessary admissions, and rebuild their confidence in familiar surroundings, without assuming they need long-term care.
Supported by the Scottish Government, HAP is part of our Home First approach – helping more people stay at home, or return there sooner, with the right support around them. It’s about preventing avoidable hospital admissions wherever possible, and when a hospital stay is needed, making sure people can leave safely and without delay. In doing so, we improve flow through our hospitals.
Home isn’t just where people are – it’s where they recover, rebuild, and belong.
