Dr. Abhishek Shukla, Senior Geriatrician, speaks with Senior Psychiatrist Dr. Nirupma Jaiswal about four common types of headache—tension-type headache, migraine, cluster headache, and trigeminal neuralgia. Headache is one of the most common complaints seen in clinical practice, yet its exact cause and type are often misunderstood. Dr. Nirupma explains that tension-type headache is extremely common and typically presents as a band-like, pressing or tightening sensation around the head. Stress, anxiety, excessive screen time, prolonged computer or mobile use, eye strain, and poor sleep may contribute. Treatment may involve lifestyle modification and, when appropriate, medicines prescribed by a physician. Migraine commonly causes a moderate-to-severe, throbbing or pulsating headache, often on one side of the head. Attacks may last 4–72 hours and can be associated with nausea, vomiting, and sensitivity to bright light. Triggers may include skipped meals, dehydration, lack of sleep, excessive sunlight, caffeine, and alcohol. Treatment depends on severity and the patient's medical history and may include analgesics, antiemetics, triptans, or preventive medicines. Cluster headache is an exceptionally severe headache, usually occurring around or behind one eye. It may cause tearing, nasal congestion, sweating, eyelid drooping, and redness or swelling around the eye. Attacks occur in clusters over weeks or months. Medical treatment can include high-flow oxygen, specific medicines for acute attacks, and preventive therapy such as verapamil under appropriate supervision. Finally, trigeminal neuralgia produces sudden, brief, electric-shock-like or stabbing pain, usually affecting one side of the face. The pain may be triggered by touching the face, chewing, speaking, or brushing teeth.
