Why are drugs so expensive in America? A big part of the answer is a company most patients have never heard of. Pharmacy benefit managers sit between the drug company, your insurer and your pharmacy - and three of them handle about 80% of every prescription in the country. The FTC found one generic drug marked up more than 7,700%, and $7.3 billion in specialty generic markups above what the middlemen paid for the drugs. In February 2026 one of the three settled and agreed to stop linking its pay to drug list prices. This is how that system works, why list prices kept rising, and what the industry says in its own defence.
