12 of the most isolated towns in West Virginia — hidden deep in the Appalachian Mountains, miles from the nearest hospital, and connected to the outside world by winding mountain roads. Some communities are nearly an hour from an emergency room. But which town is the MOST isolated? And could YOUR hometown be on this list?
Imagine living in a West Virginia town where the road home passes through a narrow, century-old railroad tunnel. Where one small community once turned to the Soviet Union for help building a bridge. Where the nearest emergency room is across a mountain or even in another state. These aren't fictional places or scenes from a movie. They're real communities where families have lived for generations, surrounded by some of the most remote landscapes in Appalachia.
In this Hollow Roads documentary, we're counting down 12 of the most isolated towns in West Virginia, exploring their forgotten history, difficult roads, abandoned railroads, former coal mining communities, and the extraordinary people who continue to call these places home. Our ranking is based on driving time to the nearest hospital emergency room, using the fastest available routes. And the farther we travel into these mountain hollows, the more surprising the stories become.
Some of these places once had busy railroad stations, hotels, company stores, schools, and thriving industries. Today, many of those businesses are gone. Railroads have disappeared, coal mines have closed, and narrow roads remain the only practical connection to the outside world. One community nearly vanished as mining operations expanded. Another still carries the memory of a catastrophic flood. And one former coal town has a remarkable history of fighting for the only safe way across a river.
But the most unsettling question is what happens when every minute matters. When a family needs emergency medical care, how far must they travel? What happens when mountain weather, flooding, or a blocked road makes an already difficult journey even harder? These are questions that take on a very different meaning when the nearest emergency room is dozens of miles away.

And which of these isolated places would you still be proud to call home? The final stop reveals a town where even the fastest routes to emergency medical care take roughly an hour.
These aren't abandoned ghost towns. People still live here, maintain their homes, remember their neighbors, and preserve their communities' histories. Behind every winding mountain road is a story of hardship, independence, determination, and the deep connection families have to the places they call home.

ABOUT THE RANKING: Driving times are estimates calculated using OSRM/OpenStreetMap routing data in October 2026, ranging from approximately 31 to 64 minutes to the nearest emergency room. They represent estimated fastest driving routes, not ambulance response times. Actual travel may take longer because of weather, road conditions, traffic, or closures. Population and economic data are drawn from U.S. Census Bureau records and American Community Survey estimates, with county-level figures used where appropriate.
This documentary explores the geography, isolation, and history of these communities with respect for the people who still call them home.
