Mephentermine — marketed as Termine — is a sympathomimetic amine that has quietly become one of the most widely used vasopressors in Indian anaesthesia practice, despite being largely discontinued in Western countries. Structurally related to methamphetamine, it acts predominantly as an indirect sympathomimetic, stimulating the release of norepinephrine from sympathetic nerve terminals to produce mixed alpha- and beta-adrenergic effects — increasing cardiac contractility and heart rate through beta-1 stimulation while causing peripheral vasoconstriction through alpha activity, and uniquely dilating coronary, cerebral, splanchnic, and renal vascular beds even as it raises systemic blood pressure. Its onset of action is immediate with intravenous administration and 5–15 minutes with intramuscular injection, with a duration of roughly 30 minutes intravenously and up to 4 hours intramuscularly — making it a versatile agent for both prophylaxis and treatment of hypotension following spinal and general anaesthesia. Multiple randomised trials comparing mephentermine with ephedrine and phenylephrine for spinal anaesthesia-induced hypotension in caesarean section have consistently shown comparable efficacy in maintaining maternal blood pressure with similar neonatal outcomes on Apgar scores and umbilical cord blood gas analysis, though ephedrine may achieve a slightly faster and more sustained pressor response while mephentermine has been found to be considerably more potent on a milligram-for-milligram basis. In this comprehensive clinical review, Dr. Madhuri Patil takes anaesthesiologists through the complete pharmacological profile of mephentermine — from its mechanism of action to its clinical applications, comparative evidence, dosing, and safety considerations — making this an essential refresher for a drug used in operating theatres across India every single day.
This session is essential for anaesthesiologists, obstetric anaesthesia practitioners, residents, and all clinicians who routinely use vasopressors to manage perioperative hypotension.
