This video teaches Learning Outcome 2 of Unit 4, Promoting a Positive Health and Safety Culture, in the OTHM Level 6 Diploma in Occupational Health and Safety. You will learn how the HSE's HSG48 model of job, individual and organisation explains behaviour at work, and how attitude, aptitude, motivation and background shape what people do. We explain how people perceive risk, using Slovic and Tversky and Kahneman, and how Reason's classification separates slips, lapses, mistakes and violations. You will see how job factors and organisational factors change human reliability, with a worked example of error probability across a task, and what the Piper Alpha inquiry and the BP Texas City investigation show about handover, fatigue and staffing. We then cover methods of improving individual reliability, including fatigue management and focused line management, and evaluate behaviour change: behaviour-based safety, the COM-B model, stages of change and the evidence for and against. It is for safety officers, supervisors and managers working towards CertIOSH, and for learners writing Task 2 of the OTHM brief.
