$45M Botox Medicare Fraud As Glendale Doctor Gets 15 YEARS After Billing From Cabo Beach

A federal judge in Los Angeles has sentenced Glendale doctor Violetta Mailyan, 45, to 15 years in prison for billing Medicare more than $45 million for Botox injections that prosecutors say were medically unnecessary or never provided. Mailyan owned and operated Healthy Way Medical Center. According to the Justice Department, she billed more than $19 million for injections on days her clinic was closed, billed for a patient who was in federal prison at the time, and billed while she was on vacation in Cabo, Maui, Las Vegas, Pennsylvania and New York, including over $200,000 billed while she was in Cabo. A jury convicted Mailyan in May 2026 of nine counts of wire fraud and three counts of obstructing a criminal investigation after she altered patient records and gave them to federal agents. She was sentenced on Oct. 29, 2026, ordered to pay $32,904,049.41 in restitution to Medicare, and ordered to forfeit more than $16 million in seized property. The jury found that a Tesla Model X, a Tesla Cybertruck, bank and brokerage accounts, and properties in Surfside and Glendale were proceeds of the fraud. The Justice Department says she also bought a $12,000 17th century crossbow and a $3,000 painting. The case was flagged by the Fraud Division's data analytics team after she was paid more than $24 million over four years for Botox, six times the next highest group of providers.
