Why Are Care Teams Still Piecing Together the Patient Story? A Conversation with Jean Ross

🚨 New Chatting with Chat Episode 🚨 How is a care team supposed to prevent the next 911 call when it can only see the last one? On the latest Chatting with Chat, I spoke with @Jean Ross, CEO and co-founder of @Primary Record. Jean shared the example of an older man who began falling at home after losing his wife. He called 911, declined a trip to the ER, and then called again. His repeated calls raise a larger question: why does he keep falling? Mobile integrated health and community paramedicine teams often see one part of the picture once they enter the home. The rest of the patient record may still be scattered across systems. This is the gap Jean is working to close through Primary Record: “Data is table stakes now. What are we doing to add value for that person looking at the data?” It is a question I carry into my work at @Care+Wear, too. I have always believed that the people living with care and the clinicians providing it have to shape the solutions built for them.
