De Quervain's Tenosynovitis Gamer's Thumb|டி குவெர்வின் டெனோசினோவிடிஸ்| Dr Shriram Krishnamoorthy In today's video, we will be talking about 'De Quervain's tenosynovitis'.De Quervain is a scientist and an orthopedic surgeon and this is the name of a condition he discovered. To be specific, in the wrist, the nerve that goes to the thumb will be painful. It will be difficult to move the wrist and there will be swelling. If these symptoms are there, it is called De Quervain's tenosynovitis. Due to the name, do not think that it is an uncommon condition. In your house, or for a person you know and many such people can have this condition. In fact, in a week, we meet 3 to 4 people with this condition. If you look at the symptoms, they will have difficulty moving the thumb. They will have pain not in the thumb, but in the wrist. Why does one get that pain? The nerve that goes to the thumb is a tunnel at the wrist. Similar to the Carpal tunnel that we saw in the previous video. When the nerve that goes to the thumb tightens at the wrist, it will not move freely. When it is unable to move freely, it can cause tightness. The pain that is caused due to the tightness is only tenosynovitis. So who can get this? If thyroid, cholesterol or sugar is uncontrolled, definitely the chances of the person getting this pain is very high. If the problem is there, how will it present itself? You will not be able to lift anything heavy. If you do, you will experience pain. When one experiences this problem, what has to be done? Definitely, the person has to consult with the doctor. The doctor has to first check you and after diagnosing if the condition is there If needed, certain blood tests can be taken. It is especially to rule out other issues. At times if there is a fat build, there could be a problem. Sometimes, if the bone itself is sharp, it can cause that pain. So, the tests are only to rule out these conditions and this diagnosis is more of a clinical judgmental diagnosis. There is a specific test for it called the Finkelstein test where the thumb is folded inside. Fold the remaining four fingers. If you have pain what can be done? Initially, your previous medical conditions like thyroid, sugar or cholesterol, post pregnancy, hormonal changes, if these conditions are there when it subsides, the pain will definitely get better. But, if it doesn't get better or if there is difficulty during that duration, there are splints called thumb spica available. Using that the thumb alone can be immobilized and the other fingers will be free to move. So, your pain will be getting better. Second thing is local sprays. The spray can be applied over the painful area. Thirdly, you can go to a physiotherapist where they will do the ultrasound therapy. Even after doing all this if there is pain, we call it resistant De Quervain's. Despite trying all the possible treatments if the pain persists, we may try an injection or we can do a release. Will this problem remain lifelong? surely not. But unless the basic conditions like sugar, thyroid or cholesterol are under control, irrespective of the injection that you take or the release that we do there are chances for it to recur.
