Sciatica or Something Else? How Leg Pain Maps to a Pinched Nerve (Plus Red Flags) If your back hurts a little but your leg is on fire, there's a reason, and it has nothing to do with your leg. I'm Dr. Omar Ramos, a spine surgeon in Minneapolis. In this video I break down how a nerve pinched in your low back sends pain, numbness, and weakness down a specific path in your leg, and how that path can hint at which nerve is involved. What you'll learn: 1. Why leg pain often feels worse than the back pain causing it The difference between dull, hard-to-pinpoint referred pain and sharp, electric radicular pain that follows a clear line 2. How pain patterns map to nerve levels — front thigh toward the knee and shin (often L4), side of the leg to the top of the foot and big toe (often L5), back of the calf to the heel and little toe (often S1) 3. The weakness patterns that tend to travel with each of those levels 4. What actually pinches a nerve — disc herniation, narrowing around the nerve root 5. The red flags that mean same-day evaluation, not watchful waiting Sensible early steps in the first few days 6. Why an MRI finding without matching symptoms doesn't tell the whole story A note on patterns: These maps are clues, not verdicts. Nerve anatomy varies between people, conditions overlap, and more than one thing can be going on at once. Real diagnosis takes history, exam, and imaging interpreted together by a clinician who has examined you. Get evaluated the same day if you have: saddle numbness (numbness in the area that would touch a saddle), any change in bowel or bladder control, or weakness that's rapidly worsening — including a foot that catches or drags.
