On the evening of the sixth of July, nineteen eighty-eight, a crew on a fixed steel platform one hundred and twenty miles off Aberdeen reached for the most ordinary decision in offshore work: restart a pump so the oil could keep flowing. Within about two hours, one hundred and sixty-seven men were lost aboard Piper Alpha, in what remains the worst offshore oil tragedy in history. None of it had to happen.
